The occurrence
On 02 July 2020, Ottawa Light Rail Transit Citadis Spirit light rail vehicle (LRV) 1113 motor bogie 3 was undergoing retrofit at the Alstom facility in Sorel-Tracy, Quebec. During the retrofit, Alstom personnel observed wheels with cracks on the outboard face of the wheel hub of wheels located in the L17 and R20 positions.

The occurrence
On , at approximately 1443 Eastern Daylight Time, Canadian Pacific Railway Company train 320-227 was travelling eastward at 46 mph on the Ignace Subdivision when 53 hopper cars loaded with grain derailed at Mile 12.8 near Ignace, Ontario. As a result, grain was released from several cars. There were no dangerous goods involved, and no fire was reported. No one was injured.

On a Canadian Pacific Railway train derailed on the Canadian National Yale Subdivision near Hope, British Columbia. An inspection revealed that 61 loaded potash hoppers had derailed. There were no injuries, and no dangerous goods were involved. The TSB is investigating.

The occurrence
On , a Canadian National (CN) Beltpack yard assignment was switching within the CN Prince George yard. The crew lost control of the movement during the switching operation. The assignment ran away at Mile 462.4, on the Prince George Subdivision. The movement came to a stop within the yard. There was no collision and no derailment. No injuries were reported.
