At approximately 1215 on 19 September 2021, westbound Ottawa Light Rail Transit (OLRT) commuter train 1121-1138 was proceeding on Track 1 when motor-bogie 2 on the trailing light rail vehicle (LRV) 1121 derailed near the middle of the Tremblay station north platform.
A cut of 103 rail cars rolled eastward, uncontrolled at the Canadian Pacific Railway Company Agincourt Yard. The cut of cars stopped when 7 cars derailed at a derail with at least 1 car blocking the main track at Mile 196 of the Belleville Subdivision. Of the cars that derailed, 2 were tank cars with dangerous goods residue and 2 carried sulfuric acid. No collision occurred, and there were no injuries or leaks.
On , a Canadian Pacific Railway Grain unit train was proceeding westward when 43 hopper cars derailed at mile 97.4 of the Brooks Subdivision in Bassano, Alberta. There were no injuries. The TSB is investigating.
On 01 December 2022, a Canadian Pacific Railway (CP) freight train was proceeding northward on the CP Weyburn Subdivision when 38 freight cars derailed in two separate derailments near Mile 119.22.
On , a Canadian Pacific Railway (CP) freight train was proceeding east on the CP Shuswap Subdivision when at mile 116.7 it struck hi-rail track unit that was unoccupied. The hi-rail caught fire. There were no injuries. The TSB is investigating.
Tacora Resources train W039 was travelling northward on the QNS&L Wacouna Subdivision when it passed a stop signal without proper authorization at the Mai, Quebec, station at Mile 128.3. The train stopped approximately 75 feet past the signal. There was no collision or derailment, and no one was injured.
On , EXO commuter train 816 on the Mont-Saint-Hilaire line was proceeding eastward at Mile 62.18 on the Canadian National Railway Company St-Hyacinthe Subdivision when it struck a stopped automobile at a public crossing in Saint-Bruno-de-Montarville, Quebec. The driver of the automobile was fatally injured. The TSB is investigating.
On , a CN loaded unit grain train hauling 104 cars was proceeding westward on the CN Wainwright Subdivision when 18 grain hopper cars derailed at Mile 244.9 Wainwright Subdivision. There were no injuries. The TSB is investigating.
On , a Canadian National Railway Company (CN) freight train was travelling westward on the Wainwright Subdivision when it passed a stop signal and sideswiped a westbound CN freight train that was departing the yard west of Wainwright, Alberta. As a result, two locomotives and three loaded intermodal cars on the first train derailed, while six loaded hopper cars on the second train derailed.
